What could cause continuous low blood sugar even after you have had over 20 oz of orange juice t?

my father has diabetes and his sugar is at 54 and then 61 ...will not go higher even though he has taken over 15oz of oj and also some regular soda trying to raise it...what could be causing it not to go up?
